  • Overview

    The Peer Support Specialist (IHOST team) position will utilize engagement strategies in order to provide community outreach to persons experiencing homelessness and mental health/substance use issues. A qualified candidate will be able to use their own recovery experience to assist others in accessing needed services and resources to support wellness.

    Responsibilities

    • Provide community/street outreach to engage persons experiencing homelessness
    • Function as part of a large multi-agency team whose focus is to assist individuals in accessing permanent supportive housing
    • Assisting clients with their recovery goals and monitoring progress
    • Accurate, timely progress notes and reports/documentation
    • Proficiency in using computer and hand held technology to manage health outcomes

    Qualifications

    • High school diploma or equivalent
    • Must be eligible to become a Certified Recovery Specialist (CRS) and a Community Health Worker (CHW) in the State of Indiana, by meeting the eligibility requirements for the CRS and CHW for the State of Indiana. Prior certification as a CRS and CHW is a plus, but CRS and CHW training will be provided to those hired.     • Must hold a valid driver’s license with acceptable driving record and provide own, insured, transportation
