Adult and Child Center wraps children and families in a full continuum of services to strengthen families, ensure the safety of each child, and help them become self-reliant. The Wrap Facilitator coordinates needed services for children and families, providing intensive case management, care coordination, and services to children/adolescents and their families. This individual works on a team with other Wrap Facilitators, therapists, and Skills providers while collaborating with external partners (including: the Department of Mental Health and Addictions (DMHA), families, outpatient providers, Department of Child Services (DCS), Division of Aging, schools, wrap provider agencies, primary care physicians, community resource organizations, and Juvenile Probation).   The Wrap Facilitator works in partnership with seriously emotionally disabled children, their families, and referral sources to facilitate high fidelity wrap services, with the goals of the child maintaining safely in the home, school and community. The Wrap Facilitator utilizes a holistic, strengths based, and family driven treatment approach to facilitate collaboration between the youth/family and all parties involved in the client's treatment, to ensure the youth/family's success and longer-term stability.   Wrap Facilitation is a dynamic position that encourages the team to utilize out-of-the box thinking in developing creative solutions. The wraparound model supports long-term stability for youth and families through a strengths-based, family-driven, team-supported, and culturally competent process. Our Skills Development Specialists work one-on-one with students in the classroom, or in a school setting, to build needed skills. They also work with students and their families/caregivers in the home to provide a full range of support.    The Skills Development Specialist works with our School-Based Teams, which also include a Team Leader, Psychologist, and Staff Therapist. Skills Development Specialists help children and teens develop behavioral health- related skills to succeed academically and socially, creating a lasting impact throughout their lives.

The Behavior Health Home Team is an integrated team of providers using a health home model. The team will include a multidisciplinary set of providers including a Clinical Team Leader, a Primary Care Physician/APRN, a Psychiatrist, a Nurse Care Manager, Team Nurse, Licensed Social workers, Skills Development Home Health Specialists, and Engagement Specialists. This team will increase consumer access to integrated mental health & primary care by using a health home model to support the recovery of this high risk population. This team will also be using innovative technology-based resources to manage health outcomes.   The Community Health Home Specialist position will work to coordinate integrated primary and behavioral healthcare services to promote continuity of care and improved health outcomes. To develop skills including self-care, daily life management, problem-solving, wellness and disease management using structured interventions for attaining goals identified in the recovery plan and the monitoring of progress in achieving those skills.
