Adult and Child Center wraps children and families in a full continuum of services to strengthen families, ensure the safety of each child, and help them become self-reliant. The Wrap Facilitator coordinates needed services for children and families, providing intensive case management, care coordination, and services to children/adolescents and their families. This individual works on a team with other Wrap Facilitators, therapists, and Skills providers while collaborating with external partners (including: the Department of Mental Health and Addictions (DMHA), families, outpatient providers, Department of Child Services (DCS), Division of Aging, schools, wrap provider agencies, primary care physicians, community resource organizations, and Juvenile Probation).

 

The Wrap Facilitator works in partnership with seriously emotionally disabled children, their families, and referral sources to facilitate high fidelity wrap services, with the goals of the child maintaining safely in the home, school and community. The Wrap Facilitator utilizes a holistic, strengths based, and family driven treatment approach to facilitate collaboration between the youth/family and all parties involved in the client's treatment, to ensure the youth/family's success and longer-term stability.

 

Wrap Facilitation is a dynamic position that encourages the team to utilize out-of-the box thinking in developing creative solutions. The wraparound model supports long-term stability for youth and families through a strengths-based, family-driven, team-supported, and culturally competent process. The holistic wrap model focuses on identifying the underlying needs of a youth/family through strengths-based discovery, and is often successful for families who have had limited success with traditional outpatient treatment.

We are currently seeking a Tutor to provide direct academic tutoring services to clients participating in Adult & Child Health’s Therapeutic Foster Care program. This position will provide intensive support regarding specialized tutoring in academic courses, baseline and benchmark assessments, regular progress monitoring, and ongoing educational planning, in the office, community, and/or home setting. Additionally, this position  will assess the needs of youth in care, communicate with foster parents and agency staff, establish individualized learning goals, prepare and deliver standards-based instructional activities, and implement necessary academic programs. This individual collaborates with foster youth, foster parents, the Education Advocate, Therapeutic Care Specialists, and other agency staff to improve the educational outcomes for their clients. 

 

The Tutor will:

  • Identify educational hurdles which impede the school progress of foster youth and assist foster youth in overcoming these hurdles
  • Develop a comprehensive plan to meet the needs of foster youth in care
  • Establish academic goals for each individual client
  • Provide specialized and direct tutoring services as scheduled
  • Assess the academic proficiency of foster youth at regular intervals, and track educational outcomes of youth in care
  • Plan and create instructional resources with clear learning objectives for use during tutoring sessions
  • Support youth in care with alternate strategies and provide additional assistance with school assignments and projects
  • Create lesson plans and modify instruction accordingly throughout the youth’s time in care
  • Encourage and facilitate parental involvement with foster youth’s education programming whenever possible and inform foster parents of family responsibilities connected with the activities
  • Create a positive educational climate for youth in care
  • Provide all statistical and program reports as needed for appropriate grant outcomes by accurately collecting and documenting necessary data in accordance with established guidelines
  • Participate in ongoing training sessions and collaborate with program coordinators to ensure initiatives are being met
  • Be familiar with and abide by the contract guidelines of the Department of Child Services and the Nina Mason Pulliam Charitable Trust Grant requirements
  • Complete other duties as assigned

This position will supervise the direct care of at least two other medical providers in 2 locations; however, the scope of responsibility is expected to increase rapidly to include more locations, more providers, and additional specialties to include pediatrics, OBGYN, Medication Assisted Treatment for addictions, and others.

 

We are investing in expanding our mission by developing high quality primary care sites in downtown Indianapolis and in the City of Franklin. This position is expected to take a clinical leadership role (25%) as well as provide direct care (75%). A successful candidate will have demonstrated experience in leadership within a matrixed environment, passion for working with disadvantaged populations, a propensity for working in multi-disciplinary teams, and an entrepreneurial spirit.  A&C expects to achieve Federally Qualified Health Center status in Q1 of 2018, so FQHC experience is a plus, however is it not required.
